I realize you are looking for templates,
but a couple of months ago I came across a little book
on writing policies, which made a related job so much easier:

Even if you find yourself saying "yeah, it's obvious"
for most of the stuff you read in there,
it's useful to have it all in writing
so you can follow the general directions.
