I've had excellant results by using cisco netflows.  They can be collected 
either from a cisco router/switch that supports it (C7206VXRs in my 
network), or via nProbe <http://www.ntop.org/nProbe/> (previous life).  Both 
of these work well for exporting netflows.

I use Flow-Tools to collect the flows and analyse them.   Using this, I've 
got graphs and reports that work great for providing useful information.  It 
also makes it easy for me to calculate 95%ile for a few colocated boxen I have.

Currently, we do very little in the way of bandwidth shaping and 
restricting.  Most of what does occur in that is rate-limiting from the 
cisco routers.
