There are any number of standard VoIP/SIP clients.  One that I have 
used in the past is the Gizmo Project, but it had some problems with 
the latest version of the Apple 802.11pre-draft2n squarish Airport 
Extreme router/access points, and I don't know if they got that 
problem fixed.

There's lots of standard H.264 software here, too.  On the Mac, you'd 
obviously use iChat.  And some of the big-name AOL & Yahoo! chat 
implementations are video-enabled, too.
